Fines Exemption Form

Need to know more information about paying fines? See Fines.
Once your request is submitted, you will receive a written response by email within 2-3 working days. 

Request for Fines Exemption

Please explain why you feel that you should be exempt from paying the Library fine. 

File Submission

You should submit a copy of any medical certificates or other supporting documentation ie medical certificates, copy of police report in case of theft etc using the file submission option. 

All forms submitted are assessed by the Fines Officers. 

If you do not want to submit supporting documentation on this form please supply a copy to the Library Loans Desk. 

The maximum file size of upload is 25 MB. Use Zip if you want to submit multiple files. For help please call the Library on 02 9514 3666.

I have a court order pdf file. I couldn't upload multiple files here. It didn't even allow me to upload a zip file as there is mentioned there.

Please let me know if you need the court order file.
