Open your document and click on (Word 2003 or on a Macintosh) the Remove Field Codes icon in Word's EndNote toolbar; or (Word 2007 or 2010) the EndNote tab, then Convert Citations and Bibliography, and Convert to Plain Text. This creates a copy of your document without these EndNote codes. Save and submit this copy to your publisher.
The original document (with all the EndNote codes intact) is unaffected and this original is what you must use if, subsequently, you wish to insert more citations or do further editing.
